Camper Information
This section of the registration form is where the general information of the camper is requested. The fields that are required are as follows:

The Grade Entering menu is to select the grade the the camper is going to be going into when they return to school in the fall. For example, if a camper finished the 4th grade at the end of the school year and will be entering the 5th grade in the fall, they will 5th graders at camp and will be in the 4-5 Grade camp.
Address: Please enter the address where the camper resides. This is where the camper lives throughout most of the year. Two lines are provided to the convienience of those who use two line for their address. This address will be used to send out camp registration information for the next year. Address Line 2 is not required for registration.
Phone Numbers: Please enter the home phone number of the address listed above. If you choose to enter a cell phone number, enter the number of the person who can be contacted in case the home phone number is not available. A cell phone number is not required for registration.
E-mail address: Please enter a valid e-mail address that you can be reached at in a timely manner. We will use this e-mail address to contact you about question with your registration information and about payment information and options. If you do not have a valid e-mail address, please enter a valid phone number where you can be reached regarding the same information. This address will not be used for anyother purpose, nor given to person or company without permission.
Church Information: If you attend a church on a regular basis, please enter the information here. CHYC will use this information to contact the church about future camping opportunities and camping information.
Cabin / Camper Reguest: These boxes are for the convinience of the camper. If a camper has a specific cabin that they would like to stay in, or another camper that they know that they would like to bunk with, please enter that information here. Please note that all requests will be taken into consideration only with preregistered campers. Also, cabin requests may not be given depending on number of campers and open cabins for the week. These fields are not required for registration.
Family Camp: This form allows for an entire family to register without filling out multiple forms. Please enter all the information for every family member. If your family lives at two different addresses, plese enter the address where the majority of the family members live, or where future camping information can be sent. Families will stay in the same cabin with each other, so we allow cabin requests for each family. Please note that preferences are given to preregistered campers only. Families may share a cabin with another family.
Adventure Camp: Because Adventure Camp does not take place on the CHYC camp grounds, no cabin or person request will be taken since they are not needed. All other areas apply.
Women's Retreat: Gender and Grade Entering fields are not available because they are not applicable for this retreat. Cabin requests are not available for the retreat, but person requests are. Special cabin arrangments will be handled on arrival to CHYC.
Financial Information
This section of the registration form is where the monetary specifics of the camper are identified and indicated. This section will be checked by the camp managers for accuracy. If there are any inaccuracies with the information given, you will be notified of the changes. The fields that are required are as follows:

Camp Fee: The camp fee field is automatically filled out on all forms (except Family Camp). These are the actually camping fees for that particular camp and do not change. This filled out for your information and convinience.
Camp T-Shirt / Photo CD Qty: These fields are to be filled out with quantity for each that the camper wants. If the camper is not interested in either of these, please insert zero into the appropriate box. You will be charged for each item you specify. For example, if you insert "2" for the t-shirt box and "3" for the photo CD box, you will be charged for two t-shirts at $12 a piece and for 3 photo CDs at $10 a piece. Please insert only the amout that the camper desires.
Money for Mission: Each camp has a particular mission that it will be raising money for. Campers are encourages throughout the week to give money for this mission. Please enter the amount of money in this field that the camper would like to give to the mission of that week. If the camper desires to give more money than is designated here, they may take money out of what is designated for canteen. This field is given for the convienience of the camper and is not a required field.
Money for Canteen: Everyday at camp, campers have an opportunity to purchase refreshments at the camp Canteen. This is money to be set aside for the camper to use for canteen. CHYC uses a card system when campers purchase items from the canteen. The campers are not allowed to carry money on them to help reduce the opportunity for the theft of their money. If the camper so chooses, they may transfer some money from their canteen card to missions if they choose to give more money than what was designated for mission. This field is given for the convienience of the camper and is not a required field.
Other / Other Amount: If there is any other reason that the camper is paying money along with their registration fee, please specify the reason in the Other field, and then the amount in the Amount field. This field is given for the convienience of the camper and is not a required field.
Church Scholarship / Amount: Some churches who are associated with CHYC offer campers from their churches different scholarship opportunities. If you are recieveing a scholarship from your church, please enter the church name in the Church Scholarship From field and the amount of the scholarship in the amount field. CHYC will contact the specified church for payment of the scholarship. Please note that this amount will be subtracted from the final amount.
Total Due: This field is where the final amount to be payed to CHYC will be inserted. Please use this formula to figure out the final Total Due:
Camp Fee + T-Shirt (Qty x $12) + Photo CD (Qty x $10) +
Other Amount - Scholarship Amount + Late Fee (if applicable) =
Total Due
This is the amount that is due at the beginning of camp. Please note that this amount is to be paid in full two weeks before the specified week of camp is to start to avoid a $10 late fee.
Family Camp: Family Camp adds three new boxes to be filled out: adult, youth and children. In these boxes, please specifiy the number of each you have in your family. Adults are ages 13 and up, youth are ages 6 - 12, and childres are 5 and under. To figure out the Camp Fee for family camp, please us the following fomula:
Adults (Num x $75) + Youth (Num x $45) = Camp Fee
Please note that those 5 and under are free. Also note that the max that the Camp Fee can be is $220.00. For example, if your family consists of 4 adults over 13, the camp fee is $220.00 even though 4 adults at $75 a piece is $300.00. You will pay no more than $220.00 for Family Camp Fee.
Adventure Camp: Because Adventure Camp does not take place on the CHYC camp grounds, the field to enter amount for canteen is not available. All other areas apply.
